    Our organization started to use CHF Solutions (Aquapheresis) back in December 2008. To date we have used it on 90+ patients. We are hoping to have an OP component for Aquapheresis & thus decrease unnecessary hospital readmissions.  The DOH is currently here at our facility so we should learn this week if we are approved to have the OP offering.  
     
    Overall, we are hoping that Aquapheresis therapy can help us cut down on readmissions, and, if initiated early enough in a patient’s hospital stay, perhaps even impact LOS.
     
    That being said, we believe Aquapheresis is only a piece of the complex puzzle in the effort to impact readmissions.  We believe the early follow-up appt., f/u phone calls, enhanced communication, collaborative rounds, etc. need to all be in place to drive down readmits.
