This group is primarily charged with identifying the tools and best practices most likely to be modeled in effort to establish 'positive deviance' and to spread their innovations to reduce readmission rates.
Responsibilities Include:
-Developing guidance on how to create an improvement team with community connections
-Developing guidance on how to develop, implement, and monitor a participant-specific improvement plan
-Identifying the Best Practices from among the other national initiatives working to reduce readmission rates
-Assembling a menu of Tools on the H2H website
-Creating a repository of Best Practices on the H2H website
