About H2H

The H2H initiative, cosponsored by the American College of Cardiology and the Institute for Healthcare Improvement, is an effort to improve the transition from inpatient to outpatient status for individuals hospitalized with cardiovascular disease.

About H2H
H2H is designed to leverage the talent, energy, vision and accomplishments of partners across the health care system to provide guidance about how best to improve care.

 

Leadership
H2H is advised and led by experts and H2H program participants committed to improving patient care and outcomes.

 

Strategic Partnerships 
The H2H Strategic Partnership is a partnership of trade organizations, medical societies, quality improvement organizations, and other health care orgnizations that are committed to improving care transitions, empowering patients, and reducing preventable and costly hospital readmissions.

  Participants 
H2H Particpants include individuals and organizations across the healthcare continuum, including hospitals, practices, home health agencies, community leaders, and more.  See state listing of H2H participating facilities.
