What is H2H?
The Hospital to Home (H2H) national quality initiative is an effort to improve the transition from inpatient to outpatient status for individuals hospitalized with cardiovascular disease. The H2H project aims to create a rapid learning community where people can share their knowledge and best practices regarding this critical transition.
Rather than imposing and advocating specific strategies, the H2H project provides a central clearinghouse of information and tools, building on what others are doing and have done to improve care transitions and readmissions. H2H will focus on three main domains that provide opportunities for improvement:
(1) Is the patient familiar and competent with their medication and do they have access to them?
(2) Does the patient have a follow-up visit scheduled within a week of discharge and are they able to get there?
(3) Does the patient fully comprehend signs and symptoms that require medical attention, and know who to contact if they occur?
